Note generalized pitting throughout pipe as well as severe corrosion around flanges.
Some pits have extended through the pipe, resulting in complete pipe failure.

RE: Recommendation for Award of Cullen Lift Station Modifications
Mr. Cameron:
On Thursday May 27, 2010, bids were received and opened for Cullen Lift Station
Modifications. Five bids were received with the apparent low bidder being Matula & Matula
Construction, Inc.with a bid total of$57,054.00. All of the bids were reviewed for accuracy, and
no mathematical errors were found. See the attached bid tabulation for more details.
During the review of qualifications provided by Matula & Matula Construction, Inc., we found that
they are qualified to complete this work in a timely manner; however, we also discovered that
they are currently under investigation by OSHA for a work site fatality. The fatality is still under
investigation by OSHA and the cause of death has yet to be officially determined. Since the
incident, Matula & Matula Construction, Inc. has taken measures to prevent another such
incident by hiring a Safety Diector full time as well as re-writing their safety guidelines. Matula &
Matula Construction, Inc. supplied a copy of their revised safety guidelines for review.
During our investigation Matula & Matula Construction, Inc have been forth coming and willing
to assist and provide documentation for our consideration. Please find the attached Safety
Guidelines provided by Matula & Matula Construction, Inc. Also attached is a letter from the
Safety Director stating that they will follow the attached guidelines as well as that he shall
inspect the site regularly to ensure that their employees are adhering to the plan.
Considering all the information above, we recommend award of the contract to Matula & Matula
Construction, Inc. If you have any questions or would like more information, please let me know.

workplace free from recognized hazards, but also strict adherence to the safety standards set forth in
our own M&M Construction Safety Manual.
Given the opportunity on the Cullen Lift Station Modifications Project,we assure you that our
workforces will be equipped with the training and resources necessary to execute all assigned work
in accordance with our written safety program which has been submitted to Jacobs Engineering.
Furthermore,to ensure workforce safety and compliance with safety standards our in-house Safety
Director will inspect the jobsite and work practices on a weekly and as needed basis.
