Five bids were received with the apparent low bidder being Matula & Matula Construction, Inc.with a bid total of$57,054.00. All of the bids were reviewed for accuracy, and no mathematical errors were found. See the attached bid tabulation for more details. During the review of qualifications provided by Matula & Matula Construction, Inc., we found that they are qualified to complete this work in a timely manner; however, we also discovered that they are currently under investigation by OSHA for a work site fatality. The fatality is still under investigation by OSHA and the cause of death has yet to be officially determined. Since the incident, Matula & Matula Construction, Inc. has taken measures to prevent another such incident by hiring a Safety Diector full time as well as re-writing their safety guidelines. Matula & Matula Construction, Inc. supplied a copy of their revised safety guidelines for review. During our investigation Matula & Matula Construction, Inc have been forth coming and willing to assist and provide documentation for our consideration. Please find the attached Safety Guidelines provided by Matula & Matula Construction, Inc. Also attached is a letter from the Safety Director stating that they will follow the attached guidelines as well as that he shall inspect the site regularly to ensure that their employees are adhering to the plan. Considering all the information above, we recommend award of the contract to Matula & Matula Construction, Inc. If you have any questions or would like more information, please let me know.
