The contract amount was valued at the amount of work to be accomplished on sidewalk repair and replacement. Four locations have been identified by staff for pavement repairs via panel replacement within the Country Place subdivision, which was a type of repair and maintenance work included in the contract. These pavement sections were identified with a Pavement Condition Index of 61.5, which requires replacement. Existing bid items were included in the bid and via this amendment be quantified and valued for concrete street panel removal and replacement. Additional bid items are also necessary and were identified and required after Greenscapes Six reviewed the area with staff. The items were minor in cost and scope and were inadvertently left out of the original bid document, but are necessary for the pavement replacement—such as traffic control at intersections and a fast setting subgrade as provided by the use of cement stabilized sand instead of bank sand lime stabilized subgrade. The current contract includes bid items for: Saw Cut Concrete, Removal & Replacement of 8" Concrete, and Removal & Replacement of 6" Curb. The additional quantities for these existing bid items is $85,310.66. The additional quantities and value of existing bid items for Fiscal Year 2018 are included below. New bid items are required for: Traffic control, Removal of Subgrade, Plate Compaction of subgrade, Install 6 in Cement Stabilized Sand. The value for these existing bid items is $18,435.54. The new bid items and quantities for Fiscal Year 2018 are included below. This total amendment encompasses the following additional quantities for existing bid items, in the amount of$103,746.20, needed to repair the four pavement locations in the Country Place subdivision. The duration of this work is expected to be 60 days from the notice to proceed. 1 of 2 D6.Revised 6/16 Description of Work: FY2018 Existing Items-Additional Quantities Unit Price Qty Amount Saw cut concrete full depth-LF $ 18.00 228 $ 4,104.00 Remove&Replace 8"pavement-SY $ 127.50 605 $ 77,066.66 Remove&Replace 6"curb-LF $ 23.00 180 $ 4,140.00 Subtotal Existing Items Add.Quantities $ 85,310.66 New Bid Items and Quantities(Not in contract) Unit Price Qty Amount Traffic control -SF $ 1.25 5,440 $ 6,800.00 Removal of subgrade-CY $ 14.00 101 $ 1,410.36 Plate compact subgrade-SF $ 0.25 5,440 $ 1,360.00 Install 6"Cement Stabilized Sand -CY $ 80.00 111 $ 8,865.18 Subtotal for New Bid Items&Quantities $ 18,435.54 Grand Total for FY18 $103,746.20 Back-up documentation is attached: Greenscapes Six proposal dated July 6, 2018 (attached).
